Athletic Placement Process (APP)

Must be recommended by the coach to participate at the high school level of competition. Contact the athletic office with any questions.

Physical Form: To be completed once per year by a physician.

Dental Health Certificate

Parental Permission Form for Physical Exam by School Doctor (With Opt Out): To be completed by parent/guardian once per year.
