Athletic Placement Process (APP)

Must be recommended by the coach and pass all physical requirements to participate at the high school level of competition. Contact the athletic office with any questions.

Forms

Physical Form: To be completed once per year by a physician.

Dental Health Certificate

Parental Permission Form for Physical Exam by School Doctor (With Opt Out): To be completed by parent/guardian once per year.

Transportation Release Form
